Can I leave a message for a patient if I do not have their patient code?

Yes, we can take a message for a patient, but we cannot confirm or deny if a patient is in our care.

How can I request my medical records?

We appreciate you receiving care with us. Medical records requests should be submitted to Via Linda’s Health Information Management (HIM) Department. For more information or to submit a request, they may be reached by phone (480) 476-7134 or fax (480) 476-7133.

What are your visitation hours?

The visitation schedule and guidelines can be viewed here. Please note, these hours are subject to change.

Can I speak with a provider about my adult child’s (18 years or older) care?

We can take in information but if a patient has not signed an official Release of Information to grant you access to their medical records, we are unfortunately unable to provide care information to you unless you have legal Guardianship of your loved one and have provided legal documentation.

What is HIPAA?

Information about your health is private. And it should remain private. That is why Via Linda Behavioral Hospital is required by federal and state law to protect and maintain the privacy of your health information. We call it “Protected Health Information” (PHI).

The basis for federal privacy protection is the Health Insurance Portability and Accountability Act (HIPAA) and its regulations, known as the “Privacy Rule” and “Security Rule” and other federal and state privacy laws.
