Yes, we can take a message for a patient, but we cannot confirm or deny if a patient is in our care.
We appreciate you receiving care with us. Medical records requests should be submitted to Via Linda’s Health Information Management (HIM) Department. For more information or to submit a request, they may be reached by phone (480) 476-7134 or fax (480) 476-7133.

We can take in information but if a patient has not signed an official Release of Information to grant you access to their medical records, we are unfortunately unable to provide care information to you unless you have legal Guardianship of your loved one and have provided legal documentation.
